SUMMARY
Evaluates and develops an Individualized Educational Plan (IEP) to meet the specific needs of students who qualify for physical therapy services as diagnosed via referral for special needs programming.

MINIMUM REQUIREMENTS

• Arizona Physical Therapy License
• Verbal & Written communication skills in English and a demonstrated ability to read and comprehend written/graphic and oral instruction
• Must release Social Security Number to be eligible for AHCCCS-Provider ID Number

REQUIREMENTS PRIOR TO START DATE

• Arizona IVP fingerprint clearance card
• Proof of immunity to rubeola (measles) and rubella (German measles), or proof of MMR immunization
• Valid Driver's license (Arizona Driver’s License required within 30 days of hire)
• MVReport – showing no more than the allowable points against Driver’s License as described under Governing Board Policy: EEB-R-1 Business and Personnel Transportation Services - Transportation by Employees.

Location: Exceptional Education Department - District Wide - Sites May Vary - 1 position available 
FTE: 1.0 - 7.5 hours per day
Work Calendar: 9.5 month
