Occupational Therapist
SUMMARY
Evaluates and develops an Individualized Educational Plan (IEP) to meet the specific needs of students who qualify for occupational therapy services.
MINIMUM REQUIREMENTS
- Arizona Occupational Therapy License
- Arizona IVP fingerprint clearance card
- Verbal & Written communication skills in English and a demonstrated ability to read and comprehend written/graphic and oral instruction
- Must release Social Security Number to be eligible for AHCCCS-Provider ID Number
ADDITIONAL REQUIREMENTS AFTER HIRE
- Proof of immunity to rubeola (measles) and rubella (German measles), or proof of MMR immunization
- Valid Driver's license (Arizona Driver’s License required within 30 days of hire)
- MVReport – showing no more than the allowable points against Driver’s License as described under Governing Board Policy: EEB-R-1 Business and Personnel Transportation Services - Transportation by Employees.

- Work Calendar: 9.5 month
- Positions less than 30 hours per week are not eligible for district benefits.
- Salary will be prorated if the start date is after the first day of the work calendar for the position.

Probationary Contracts will be issued for any contingent Certifications, Positions that are 0.6 FTE or less, and certified employees within their first three years with the District.
